The backstab would just be a normal attack until the weapon makes contact with the other player. Dodging it would be no different than dodging any other R1. Also, Demon's Souls might of had the server act as a referee when it came to determining who dealt damage. Dark Souls doesn't have a third computer in the equation to do that.

Backs have quite a big window. You can be slightly to their side and still backstab. Unlocked makes positioning easier. Latency also plays a part. You need to press R1 at their back, but if they stay in place (regardless of direction) you'll still pull them into a backstab. This makes 'face stabbing' a thing.

Damaging someone right as they go to backstab you will cancel all damage. You will be put into the animation but receive no damage. I was talking to unbound. Assuming he is talking about backstab escapes, they are done by pressing R1 to backstab your opponent 1-frame after your opponent inititates a backstab.
